Facility Evaluation Report

On 07/28/26 Licensing Program Analyst (LPA) Mario Leon conducted a case management visit to deliver one (1) deficiency, related to California Department of Social Services (CDSS) observation on 05/26/26. CDSS has observed that one (1) Staff (S1) did not produce any Special Incident Report (LIC624) related to R1 being relocated.

There has been one (1) deficiency delivered during today's visit.

An exit interview was held with Shirley Danting - Administrator, and a copy of one (1) deficiency, facilities' appeal rights and this report have been provided.
