Complaint Investigation Report
The investigation revealed the following: Allegation-Staff members consumed alcohol/drugs during work hours, impairing their ability to provide adequate care and supervision, which presents a risk to clients in care.
The details of the complaint alleged that the facility staff are smoking marijuana in the backyard at night, while caring for the clients at the facility. On 12/2/2025, from 9:20am-3:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation and stated that they have never used any type of alcohol or marijuana at the facility. S1 stated that staff are not allowed to use any type of drugs or alcohol while working with the clients. S1 further stated that it is a violation of Trinity’s Homes II policy to be in possession, use, sale, or be under the influence of drugs or other controlled substances during working hours and caring for the clients.
The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that they have never observed any of the staff at the facility drinking alcohol nor have they observed them smoking marijuana. All clients stated that they feel safe living in the facility and felt the staff provided them with adequate care and supervision. They further stated that they had no complaints or concerns about the staff at the facility nor their ability to care for them.
The department reviewed the House Values (No Date) and the Staff Rules/Mission Statement ( Signed & Dated: 09/01/2023, 02/17/2025, 07/01/2022) that were posted in the facility and observed that the rules governing staff behavior were posted and signed by staff. In this posting it states, the possession, use, sale, or being under the influence of drugs or other controlled substances or alcoholic beverages during working hours or on the premises at any time is a violation of Trinity Homes II policy and will not be tolerated.
Based on interviews and records reviewed, there is insufficient evidence to support the allegation that Staff members consumed alcohol/drugs during work hours, impairing their ability to provide adequate care and supervision, which presents a risk to clients in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated .
An exit interview was conducted with Oluwarotimi Johnson (Direct Support Professional), and a hard copy of this Complaint Investigation Report was provided.
