Facility Evaluation Report
On 05/16/25, at 2:10pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced annual required inspection visit to Threenity Homes. LPA met with Babatunde Babashola, Direct Support Professional, and Oluwarotimi Johnson, Facility Manager, and explained the purpose of today’s visit. The facility is licensed to serve (4) Developmentally Disabled clients ages 18 to 59. Currently, the home has (4) clients. The clients are South Central Los Angeles Regional Center clients. None of the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices. The facilities annual fees are current.
The facility is a two-story residential home located in a residential neighborhood. The home consists of the following: 4 bedrooms, 2 bathrooms, family room, dining room, kitchen, living room, shaded area, indoor and outdoor activity area, laundry room and an attached garage.
LPA conducted a records review of (4) client records, (4) staff records, (4) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (3) Client Medication Administration Records and medication and did not observe any discrepancies at the time of visit. The last client does not take any medication.
At 2:20pm, LPA and staff toured the physical plant. There are no bodies of water or firearm/ammunition on the premises. All client rooms were checked. Beds and bedding were in good condition, adequate lighting provided, adequate storage for client personal belongings was observed. Walls and floors were in good repair.
