The inspector’s account

On 10/15/25, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Trinice Levias as the purpose of the visit was explained . The facility is licensed for a capacity of 4 adults ages 18-59, ambulatory only. There are currently (0) clients living at the facility. Facility fees are current.

The facility is a single story home located in a residential neighborhood that consist of the following: (4) bedroom, (2) bathroom of which (1) is private, a living room, a kitchen with a dining area, a patio, and an attached garage that houses a washer and dryer, and will serve as a storage space. The Facility walls and floors were in good condition. All client rooms were checked, beds and bedding were in good condition, adequate lighting provided, adequate storage for client personal belongings was observed. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries available for clients. The water temperature measured between 105 F.- 120 F.. A Land line and internet service were observed, a supply of perishable and non-perishable food supply was observed. There are no bodies nor obstructions on the premises, walkways throughout the day program and all exits were clear of hazards and debris.

Chemicals and sharps were properly locked, first aid kit was fully stocked, smoke alarms and carbon monoxide are hardwired and operational. LPA observed (2) fire extinguishers were observed throughout the facility.

Exit interview conducted, and a copy of this report was provided.
