Complaint Investigation Report
The investigation revealed the following: Allegation #1- Staff inappropriately touched client in care.
The details of the complaint alleged that the facility staff touched client inappropriately while in care. It was reported that staff often gave inappropriate hugs to client while at the facility. On 4/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff inappropriately touched client in care. All staff stated that they have never witnessed nor participated in any type of inappropriate behavior with any of the clients in the facility. Staff also stated that they have never touched, hugged, or did anything of an intimate nature that would cause any of the clients in the facility to feel uncomfortable with their actions.
The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that they have never been touched inappropriately or have been made to feel uncomfortable by any of the staff working at the facility.
Based on interviews conducted, there is insufficient evidence to support the allegation that Staff inappropriately touched client in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated .
Allegation #2- Staff did not provide reasonable privacy to client in care.
The details of the complaint alleged that the facility staff did not provide reasonable privacy for clients while in care. It was reported that the facility had hidden cameras in the clients’ rooms. On 04/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff did not provide reasonable privacy to client in care. All staff stated that the facility does not and has never had hidden cameras in the clients’ rooms. Staff also stated that they are aware of the clients’ personal rights and have never impeded upon their privacy in any way.
The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that the facility staff does ensure that they have their privacy while at the facility and have never felt that they were being monitored in any way.
The department toured the facility and the clients’ bedrooms and did not observe any cameras or monitoring equipment in their rooms.
Based on observation and interviews conducted, there is insufficient evidence to support the allegation that Staff did not provide reasonable privacy to client in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated .
