Facility Evaluation Report
On 10/28/2025 at approximately 9:00 AM, Licensing Program Analyst (LPA) Jose Anguiano conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Rogelio Romero. The facility is licensed to operate for (6) ambulatory adults, ages 18 through 59. At the time of the visit, there were (2) clients in care.
LPA toured the physical plant. The facility is a single-story building located in a residential neighborhood and consists of (3) client bedrooms, (2) common bathrooms, a living area, dining area, kitchen, and a garage used for storage. There were no bodies of water or obstructions observed on the premises. All rooms were inspected; beds and bedding were in good condition, adequate lighting was provided, and storage for client personal belongings was available. Bed linens, comforters, and bath towels were adequately stocked. The facility was observed to be sanitary and appropriately furnished. LPA observed that the backyard shed used for storing personal hygiene items, cleaning supplies, and toxins was unlocked at the time of the visit, though it had a lock and key. No clients were present, but the area was accessible. The kitchen was inspected and found to have enough perishable and non-perishable food, all properly stored. The drawer containing sharps was also unlocked while staff were cooking. Technical assistance (TA) was provided to the administrator to ensure all hazardous items, including sharps and toxins, always remain locked and inaccessible to clients. A fire extinguisher was charged; smoke detectors and carbon monoxide detectors were tested and found to be operable. A review of the Medication Administration Records (MARs) and bubble pack medications indicated they were maintained in order and accurate.
