Facility Evaluation Report

On May 29, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced case management visit to the facility in connection with complaint 11-AS-20260524160359. The LPA met with the administrator Kim Manor and explained the purpose of the visit.

During the investigation, LPA identified a staff member #2 (S2) at the facility who had Criminal Record Clearance but had not had been transferred to this facility, in accordance with Section 80019 of the Criminal Record Clearance regulations. There was no record of LIC 9182 on file.

The Department determined the facility is not in compliance with Title 22 Regulation and is cited as follows:

Section 80019 Criminal Record Clearance(f)( 1).

Based on interviews, observation, and record reviews, the licensee violated the California Code Regulations (CCR) of Title 22, Division 6, Chapter 1.

Deficiencies are issued with immediate civil penalty, and an exit interview is conducted with Kim Manor.
