Facility Evaluation Report

On 09/26/2025 at 12:59pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced required inspection visit to Heritage House. LPA was met by Unice Young, Director, and the purpose of today’s visit was explained. The facility is licensed to operate for (4) ambulatory, (Developmentally Disabled Adults) ages 18 through 59. Currently, the home has (3) clients. The clients are South Central Los Angeles Regional Center clients. None of the clients have Restricted Health Care Conditions and none utilize postural supports or protective devices. The facilities’ annual fees are current.

The facility is a single-story building located in a residential neighborhood. It consists of the following: 3 bedrooms, 2 bathrooms, family room/dining room, kitchen, living room, indoor and outdoor shaded area, laundry room and an attached garage.

LPA conducted a review of (3) client records, (6) staff records, (3) clients Personal & Incidental records and reviewed the facility disaster plan. LPA observed that clients had the following current reports in their file: Physician’s report, Admission Agreements, Behavioral Assessments, Quarterly Reports, and Regional Center Individual Program Plan (IPP). Staff records were complete with required documents and training. Clients P & I records were accurate. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (3) client medication administration records, and medication, and did not observe any discrepancies at the time of visit.
