The inspector’s account

On 10/24/2025 at approximately 9:00 AM, Licensing Program Analyst (LPA) Jose Anguiano conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Licensee Patrick Figueroa. The facility is licensed to operate with a capacity of four (4) adults, one (1) of whom may be non-ambulatory, ages 18 through 59.

LPA toured the physical plant. The facility is a single-story structure located in a residential neighborhood and consists of four (4) client bedrooms, one (1) client bathroom, one (1) staff room, one (1) staff bathroom, a living area, dining area, kitchen, office, outdoor seating area, and a garage. There were no bodies of water observed on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, lighting was adequate, and storage for clients’ personal belongings was available. Bed linens, comforters, and bath towels were present. The bathroom was operational, and the water temperature measured at 106.0°F. The facility maintained a comfortable indoor temperature of 75°F. Furnishings were observed to be in place and in good condition. Storage areas for personal hygiene items and sharp objects were secured and inaccessible to residents. The kitchen was inspected and found to have an adequate supply of perishable and non-perishable food. A review of the Medication Administration Records (MARs) and four (4) clients’ bubble-pack medications indicated medications were accurately administered and properly stored. 2 emergency kits were available in facility. Surety bond and liability insurance for the facility was on file.
