Facility Evaluation Report
On July 7, 2026, an office meeting was held to discuss Complaint 11-AS-20250714154313. The attendees included Interim Regional Manager (RM) Janae Hammond, Licensing Program Analyst (LPA) Ernand Dabuet, Executive Director Joe Goldman, and Assistant Administrator Ginger Enriquez.
During the meeting, the Interim Regional Manager (RM) reviewed the details of the complaint. On March 17 2026, the Department confirmed allegation of neglect and a lack of care and supervision. The facility staff failed to provide proper care, which resulted in the resident to sustained (3) pressure injuries.
When the findings were delivered on March 17 2026, the Department indicated that an enhanced civil penalty determination was pending under Health and Safety Code Section 1569.49(f) for Serious Bodily Injury.
The Department is currently reviewing the complaint for an enhanced civil penalty for serious bodily injury as per H&S 1569.49(f). The total civil penalty has been set at $10,000 for serious bodily injury.
During the meeting Technical Support Program (TSP) was offered to the Executive Director. At this time the Department will follow up in a later date to see if the facility wants to participate in the program. TechnicalSupportProgram@dss.ca.gov
An exit interview was conducted with Joe Goldman, and a copy of this report was provided.
