Facility Evaluation Report

On October 22, 2025, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced Case Management visit at this facility. The Program Director, Nickie Southall, greeted LPA. LPA explained to Southall that the visit was associated with an incident on October 2 through 8, 2025.

On October 13, 2025, the El Segundo Regional Office received an incident report regarding Client #1 (C1) submitted by Program Director Nickie Southall. Southall reported that Client #1 (C1) had unauthorized absence (AWOL)/elopement dates on October 2, 5, 7, and 8, 2025 , after reviewing the financial budget. LPA interviewed staff members Staff #1 and Staff #2 regarding this incident and verified that (C1) was able to elope from the facility without facility staff supervision during evening and NOC shifts. During the visit (C1) was not available for an interview. LPA reviewed (C1’s) Individual Program Plan (dated 06/17/25) and confirmed that (C1) exhibits elopement behaviors and requires constant assistance and supervision.

During the visit, LPA Dabuet collaborated with Tony Sim, Provider Relations Specialist of Harbor Regional Center, who was investigating the same incident associated with (C1).

Based on evidence gathered, interviews conducted, and records reviewed, the preponderance of evidence standard has been met; therefore, the allegation of NEGLECT/LACK OF SUPERVISION is found to be SUBSTANTIATED.

According to the California Code of Regulations (Title 22, Division 6, Chapter 8), the following deficiency has been observed, and a citation has been issued (ref. LIC 809-D).

An exit interview was conducted, and Program Director Nickie Southall was provided with a copy of the report and Appeal Rights.
