Facility Evaluation Report

On August 11, 2024, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced case management visit at Ambitions - Carson Day Program. Program Manager Cathy Nguyen greeted LPA. The purpose of the visit was to address an incident that occurred on August 6, 2025, involving Staff #1 and Client #1 at South Bay Family Home # 198320090.

On August 07, 2025, Christopher Vigil, Site Manager, reported an incident concerning Client #1 (C1) and Staff #1 (S1), which involved a physical assault at the adult residential home of South Bay Family Home. The Client #1 (C1) was interviewed regarding the incident.
