The inspector’s account

On 07/21/2025, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Annual Visit to the facility listed above. LPA met with Site Manager, Cathy Nguyen, and the purpose of today’s visit was explained. LPA was granted entry into the facility. The day program is licensed to operate for sixty (60) ambulatory Consumers, of which ten (10) may be non-ambulatory ages 18 and over. Currently the program has (30) consumers for AM and (28) consumers for PM sessions. The consumers are Harbor Regional Center and South Central Los Angeles Regional Center consumers.

Physical Plant/Structure The day program is a single-story structure located in a commercial neighborhood. It consists of the following: administrative office, consultation area, conference room, café, physical activity room, arts and crafts room, computer lab area, break room, kitchen area, laundry area, music room, salon, and (2) designated restrooms (1) men (1) women restrooms, and a changing room with a private shower. LPA and manager toured the physical plant. All walkways around the facility were observed clean, clear, and free of debris, obstructions, and hazards. There were no bodies of water on the premises.

Rooms / Restrooms All rooms were inspected and observed clean and in good repair. All rooms were observed appropriately furnished during time of visit. LPA observed an ample supply of activities, arts, crafts, games, equipment, and reading material. All rooms were observed with ample lighting. All walkways and hallways in the facility were observed clean, clear, and free of obstructions and hazards. LPA observed the activity schedule posted in the café of the facility. The water temperature in the Art Room measured 109.7-degrees Fahrenheit. The facility temperature was maintained at 70-degrees Fahrenheit. The kitchen was observed clean and sanitary. All appliances were operable and in good repair. LPA observed sharps secured in a locked drawer behind the reception desk and are inaccessible to consumers. The water temperature in the kitchen measured 113.7-degrees Fahrenheit. Bathrooms were found to be within Title 22
