Facility Evaluation Report
On August 26, 2025, an office visit was conducted with Administrator/Licensee Ashley Acosta. The purpose of this meeting was to discuss the PLAN OF CORRECTIONS(POCs) cited during annual inspection visit on August 06, 2025.
Administrator/Licensee Acosta stated being responsible for overseeing as the administrator for all homes #198320286, #198320282, and #198602219. Currently, the homes do not have an appointed Designation of Facility Responsibility LIC 308 on file with Community Care Licensing (CCL) for all group homes.
To ensure that all staff are current with Direct Support Provider (DSP) Training 1 and 2, the topic of staffing was discussed for all homes.
Staff #2 and #5 did not have current CPR/First Aid completed certificate on file.
Client #2 did not have a current medical assessment Physicians Report LIC 602 on file.
According to the California Code of Regulations (Title 22, Division 6, Chapter 1), the following deficiencies has been observed and citation issued (ref. LIC 809-D).
An exit interview was conducted with Ashley Acosta and a copy of the report was provided.
Note: *Citations not cleared by the due date will be a $100 fine assessed for each citation until it is cleared. Civil penalties will continue to accrue until Proof of Corrections (POC) are cleared. *
