Facility Evaluation Report

On September 11, 2025, at 1:30 PM, an informal office meeting was held with Administrator and Licensee Ashley Acosta. In attendance were Licensing Program Manager (LPM) Janae Hammond, Licensing Program Analyst (LPA) Ernand Dabuet along with Community Service Manager (CSM) Mercedes Lowry and Provider Relations Specialist Juan Bermudez from Harbor Regional Center. The purpose of the meeting was to discuss compliance issues with Acosta Family Homes: I, II, and III.

During the meeting LPM discussed the following concerns:

Administrative Oversight- The Administrator is overseeing 3 facilities and is not on the premises to properly manage the facilities a sufficient amount of hours per week.

Personnel Records - Incomplete personnel records.

The following are to be completed by the dates listed below: and proof submitted to licensing:

Submit a Personnel Report LIC 500 for all three residential facilities. (due 09/18/25)

Submit a Designation of Facility Responsibility LIC 308 for all three residential facilities.(due 09/18/25)

Audit all personnel records and ensure compliance with regulations. (due 10/11/25)

A training course shall be held on reporting requirements. (due 10/31/25)
