Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Constancia Bugay, and explained the reason for the visit.
At approximately 12:45pm, LPA took a tour of the physical plant. The facility is a two story building licensed to serve four (4) adults, age 18-59. Facility is vendorized by Regional Center, and caters to Level III residents. The fire clearance is for four (4) ambulatory only. Required postings were observed in the open by the living room. The smoke is interconnected. The carbon monoxide is installed by the stairway. The fire extinguisher is located at the staff work station. It was charged on November 4, 2025.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives are stored and inaccessible to the residents. Cleaning supplies are kept in the garage.
Bedrooms: There are six (6) bedrooms. Four (4) bedrooms are for resident use, and two (2) are for staff. Resident room #1 and #2 are on the first floor. Resident room #3 and #4 are on the second. Resident rooms are private. Rooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting and closet space. The two staff rooms are on the second floor.
Bathrooms: There are three (3) bathrooms. Two (2) are designated for residents' use. Both bathrooms were properly supplied and had functional fixtures and non-skid mats. Hot water temperature was measured between 116 and 120 degrees Fahrenheit. No cleaning supplies were observed in either bathrooms during the day of the inspection. The third bathroom is reserved for staff.
