Facility Evaluation Report
Licensing Program Analyst (LPA) Tuesday Cabiness conducted an annual inspection of the facility. LPA met with staff, who allowed LPA to enter. Nannette Alabi came shortly after, and everyone was informed the reason of the visit. The current census is (4); and during the visit (2) clients were program and (2) were present during the visit.
A tour of the physical plant of the inside and outside was conducted: Kitchen: The kitchen appeared clean and the appliances and fixtures were functional and operating. LPA found a sufficient amount of perishable and non-perishable food at the facility; food was properly stored. Knives and detergents were stored in locked drawers and cabinets. Properly labeled medications were locked in a cabinet near the kitchen. Bedrooms: There were four bedrooms designated for clients' use. All bedrooms were properly furnished, with bedding and linens, as well as sufficient lighting. Bathrooms: There were two bathrooms designated for client' use. Both bathrooms properly supplied with soap and towels, as well as functional fixtures. Hot water temperature was 105.8 degrees Fahrenheit. Cleaning supplies were kept in locked cabinets. Common Areas : These included the living room and dining area. The common areas appeared clean and were properly furnished. Surrounding Grounds : Entry/exits were free of obstruction. The outdoor area was clean and free of hazards. There is a swimming pool in the backyard, which was locked and secured properly. All smoke alarms were tested and function properly. The fire extinguisher was fully charged. First aide kit had Licensing requirement items. Record Review: All client files included current medical assessments, physician orders for medications and centrally stored medication records. Medications are given as prescribed. Staff files included current first aid and CPR certification as well as sufficient training documentation. Exit Interview conducted and report issued.
