The inspector’s account

Licensing Program Analyst (LPA) conducted an unannounced case management visit to the facility in conjunction with an unannounced initial complaint visit. While LPA was attempting to gain access for inspection and investigation, the following was noted:

Upon arrival, LPA rang the doorbell and knocked on the door repeatedly. A tall person wearing glasses peaked their head through one of the four windows at the top of the front door and walked away. LPA then stepped back and saw that the living room window was open. LPA looked inside and witnessed someone pacing back and forth with a portable phone in hand. LPA attempted to contact the facility multiple times, but to no avail. LPA could hear the phone ringing inside and periodically saw someone walking around inside. LPA contacted the Administrator, Mustapha Alabi, and he stated that he is "currently out of the country" and would call the facility to give instruction for granting access. LPA continued to wait and made several more atttempts to gain entry by knocking on the door, ringing the doorbell and pointing toward the door gesturing to the person inside to let LPA in. LPA contacted the Administrator again and he said that he tried to call everyone and no one was answering and that no one is supposed to be there. LPA made a few more attempts to gain entry but all attempts failed.

Due to time constraints, LPA is unable to complete the case management and/or issue citations and civil penalties.

Exit interview conducted. A copy of this report was issued.
