This is an official report of an unannounced visit/investigation of a complaint received in our office on

02/21/2023 and conducted by Evaluator Tuesday Cabiness

COMPLAINT CONTROL NUMBER: 31-AS-20230221113723

1. Clients are being unreasonably searched

2. Staff threatened clients with eviction while in care.

Licensing Program Analyst (LPA) Tuesday Cabiness met with Administrator Mustapha Alabi and informed him the reason of the visit. The following was determined:

Allegation # 1: Clients are being unreasonably searched. From 1245pm to 130pm, and from 2pm to 330pm, LPA conducted interviews. It revealed to LPA, that a client's personal belongings was stolen at the facility, and all staff and clients searched rooms to find the stolen item. Although it was reported to LPA that staff have searched client's back-packs, LPA could not find sufficient evidence to prove the allegation, therefore based on interviews, the allegation is Unsubstantiated at this time.

Allegation # 2: Staff threatened clients with eviction while in care. From 1230pm to 130pm, and 2pm to 330pm, LPA conducted interviews. It was alleged staff threaten clients with eviction, when not following house rules. From the information obtained, there were no witnesses to the allegation, therefore, based on interviews, the allegation is Unsubstantiated at this time.
