Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted an Annual Required visit and inspection of the facility. LPA met with Assistant Administrator Emily Carino. At approximately 10:40 am, with the assistance of the Assistant Administrator, LPA took a tour of the physical plant. Required postings were observed on the living's room wall. The smoke alarm and carbon monoxide detectors were tested and they function that functions properly. The fire extinguishers are located in the living area next to the kitchen and the main hallway. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked cabinet in the kitchen. Bedrooms: The facility has six (6) rooms in total. There were four (4) bedrooms designated for clients' use. All four (4) bedrooms are designated for private use. All four bedrooms, in use by clients, were properly furnished with appropriate beddings and linens with sufficient lighting. LPA observed staff room and the office room. The office room had an office desk and extra refrigerator for the perishable food.
Laundry Room: LPA observed Laundry room to be locked and washer and dryer in good repairs.
Bathrooms: The facility has three (3) bathrooms. There are two (2) bathrooms designated for clients' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured from the bathroom sink at 107.5 degrees Fahrenheit. No cleaning supplies or hazardous items were present in each bathroom during the inspection.
Common Areas : These included the living room and dining area. The common areas were properly furnished The dining room table is large enough to sit the capacity of the facility. Seating such as couches where in good repair and sit the capacity of the facility. Properly labeled medications were locked in one of the living/dining cabinets. Facility telephone was operating and available for use.
