The inspector’s account

At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced annual inspection visit. LPA met with the Program Manager, Tigran Nersesyan, and explained the purpose of the visit. The Administrator, Sima Hakobyan, arrived shortly after.

This is an Adult Day Program (ADP) licensed for capacity of forty-five (45) Non-Ambulatory Adults. There are 38 clients currently attending the program. The program is vendorized through the North Los Angeles Regional Center.

Facility maintains a comfortable temperature of 70°F. LPA observed postings such as personal rights, building permits, and the facility’s license. LPA was informed that consumers and staff enter the facility through the back gate. LPA observed all rooms were locked by fire doors with automatic exterior locks, and the facility has fire sprinklers and alarms throughout. At 11:20am, LPA observed 3 fire extinguishers at the front and the back of the facility which were fully charged and last services on 01/09/2025. Emergency exit plans were posted in each room and throughout the facility. Emergency exit plans had current location and exit routes labeled clearly. All exit paths were free from hazards and obstructions. Smoke detectors/carbon monoxide and fire sprinklers are located throughout the facility and are hardwired. A fire sprinkler system test was conducted by LAFD on 12/10/25. The facility provides no medical services and does not store medication. The program runs from 8am-3pm. The program does not provide meals but does provide snacks; the clients bring their own lunch to the day program.

The first-aid kit has been inspected which has at least the following: tweezers, scissors, antiseptic, bandages, gauze, thermometer; including a current First Aid manual. The program site is clean, sanitary, and in good condition, all furnishings are in good repair with ample lighting. Continue on LIC809-C
