The inspector’s account

At approximately 10:30am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced annual visit. LPA met with Program Manager and disclosed the reason for the visit.

At 10:45am LPA and Program Manager toured the facility inside and out.

Facility is licensed for capacity of forty-five (45) Non-Ambulatory Adults. This is a one-story building with an office, waiting room, one large activity room, five small activity rooms, conference room, weight management room, file room, computer room, break room, storage room, utility closet and three (3) bathrooms. The waiting area provided sanitizing wipes and hand sanitizer. LPA observed postings such as personal rights, building permits, and the facility’s license. LPA was informed that consumers and staff enter the facility through the back gate. LPA observed all rooms were locked by fire doors with automatic exterior locks, and the facility has fire sprinklers and alarms throughout. At 11:20am, LPA observed 3 fire extinguishers at the front and the back of the facility which were fully charged and last services on 01/09/2025. Emergency exit plans were posted in each room and throughout the facility. Emergency exit plan’s had current location and exit routes labeled clearly. All exit paths were free from hazards and obstructions. The facility provides no medical services and does not store medication.

Bathrooms: LPA observed men’s, women’s, and staff bathrooms. All bathrooms contained fully stocked liquid soap and trash cans. Staff provide paper towels and toilet paper as needed due to consumer impulse control. Facility maintains a comfortable temperature of 75°F.
