The inspector’s account

At 01:00pm, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. LPA met with the Administrator and Program Manager and explained the reason for the visit.

At 01:15am LPA conducted a tour of the physical plant and observed the following:

Facility is licensed for capacity of forty five (45) Non-Ambulatory Adults. Upon entry, LPA observed postings such as facility licence, personal rights, etc. The waiting area provided sanitizing wipes and hand sanitizer. Facility maintains a comfortable temperature of 75°F. This is a one story building with an office, conference room, storage room, utility closet, computer room, file room, one large activity room, five small activity rooms, break room, waiting room and three bathrooms. All rooms were locked by fire doors with automatic exterior locks, and the facility has fire sprinklers and alarms throughout. LPA observed 3 fire extinguishers at the front and the back of the facility which were fully charged and last serviced on 01/09/2024. Emergency exit plans were posted in each room and throughout the facility. No medications nor medical services are provided at this facility. Consumers and staff enter the facility through the back gate.

Bathrooms: LPA observed men’s, women’s, and staff bathrooms. All bathrooms contained fully stocked liquid soap and trash cans. The hot water measured between 115.8 – 116.0°F. All rooms were locked by fire doors with automatic exterior locks. Smoke detectors/carbon monoxide and fire sprinklers are located throughout the facility and are hardwired. A fire alarm initial test was conducted by LAFD on 12/20/2023.

Between 01:50pm to 2:50pm, LPA reviewed records of seven (7) clients and five (5) staff. Resident and staff records appeared to be complete and updated. LPA collected Certificate of Liability Insurance and LIC500.

Exit interview conducted and copy of this report signed and delivered.
