Facility Evaluation Report
At approximately 9:30 AM on 03/15/2022, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual visit. LPA met with Program Manager and disclosed the reason for the visit. LPA and Program Manager toured the facility inside and out.
This is a one story building with an office, conference room, storage room, utility closet, computer room, weight management room, file room, garage, one large activity room, five small activity rooms, break room, waiting room and three bathrooms. All rooms were locked by fire doors with automatic exterior locks, and the facility has fire sprinklers and alarms throughout. The facility provides no medical services and does not store medication. Consumers and staff enter the facility through the back gate.
Upon entry, LPA observed postings such as personal rights, building permits, and the facility’s license. The waiting area provided sanitizing wipes and hand sanitizer. LPA observed 3 fire extinguishers at the front and the back of the facility which were fully charged and last inspected on 01/05/2022. Emergency exit plans were posted in each room and throughout the facility. Emergency exit plans had current location and exit routes labeled clearly. All exit paths were free from hazards and obstructions. The facility’s COVID policies and practices were posted on various signs throughout the facility.
Bathrooms: LPA observed men’s, women’s, and staff bathrooms. All bathrooms contained fully stocked liquid soap and trash cans. Staff provide paper towels and toilet paper as needed due to consumer impulse control. LPA recommended facility obtain trash cans with tight fitting lids and handwashing instruction signs by all sinks.
