Facility Evaluation Report

Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted a case management visit in conjunction with complaint # 31-AS-20260410140241. LPA did a record review of resident #1 (R1)’s file and an pre-placement was not available. Per interview with administrator, stated that pre-placement was done over the phone with the consultant. It is the administrator responsibility to conduct a pre-prospective resident interview with the resident before facility admissions.
