01/28/2026 and conducted by Evaluator Michael Cava

Staff did not provide adequate supervision to the residents

Licensing Program Analyst (LPA) Michael Cava conducted a complaint visit to the facility to investigate the above allegation. LPA met with the administrator, Rhoda Gochin, and advised her of the compleint. It was reported that on or around 01/22/26, a visit was made, by a credible source, to this facility, and it was observed that there was no staff providing care and supervision to the residents in care. When this source asked the individual that responded to them at the door who they were, this individual identified themselves as a resident. When this individual/resident was asked where is staff and administrator, they stated there is no staff or administrator present. Today's investigation consisted of interviews made from 8:30am to 9:30am with the administrator, staff and five of five residents. A physical plant inspection was also made.

Although interview made with the staff and residents cannot confirm the allegation, based on the information obtained from this credible source, the allegation is Substantiated. Citation issued on the 9099D. An immediate Civil Penalty of $500 issued for the absence of supervision. Adminisrator was advised and a copy of this report issued.
