The inspector’s account

During today’s visit, LPA requested copies of resident and staff rosters. At approximately 10:20am, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected. LPA observed C1's room and got informed by the Administrator that C1's case manager from Bridges Community Treatment Services Inc found a new placement and on 6/11/2025 C1 got transferred to another facility. LPA collected Copy of the facility exit form. No clients were present during the visit, five (5) out of five (5) clients were at day program.

Allegation: Staff did not treat client in care with respect.

It was alleged that the "Staff did not treat client in care with respect" by yelling at the client. To investigate this allegation, during the initial 10-day visit made on 5/22/2025, LPA Khurshudyan conducted interviews with the Administrator, one (1) staff member, two (2) clients, and one (1) witness. LPA also reviewed facility records and obtained copies of pertinent documents relevant to the investigation. The Administrator and one (1) staff member interviewed both denied the allegation. The administrator reported that the facility staff consistently remind clients to follow house rules, treat others with respect and respect the rights of others. LPA also noted that although C1 presents significant behavioral challenges, staff continued to use de-escalation techniques, including one-on-one (1:1) meetings. The Administrator also confirmed communication with Bridges Community Treatment Services Inc. to address C1's needs for intensive behavioral intervention. Interviews with two (2) out of (6) clients who were present at the facility and were able to communicate, stated they are happy and satisfied with both the care and the staff at the facility, and denied ever being yelled at or being disrespected. Interview with one (1) witness, who also treats C1 and visits the facility regularly, denied ever witnessing staff treating clients in care with disrespect. No witnesses or individuals confirmed the allegation, nor was there a date or time provided for the alleged incident.

Based on interviews and the information obtained, there was insufficient evidence to corroborate the allegation of staff treating client (C1) with disrespect. Therefore, the allegation is deemed UNSUBSTANTIATED at this time.
