Licensing Program Analyst (LPA) Tihesha Smith conducted a case management visit in conjunction with complaint # 31-AS-20260420105031. Interview with staff #2 (S2) and licensee revealed the following: three (3) master resident records were released to two (2) separate facilities instead of copies being provided from April 10 2026 to April 11, 2026 and has not been returned. The licensee/administrator was not notified are aware of the incident until today's visit.
