Facility Evaluation Report
Licensing Program Analyst (LPA) Abeye Duguma met with Program Supervisor, Emma Pinto, for a One (1) Year Required visit for this facility. LPA explained the reason for the visit. A tour of the physical plant was conducted at 9:00am and the following was noted:
There are two entrances being utilized at the facility. The facility is fire cleared for forty (40) ambulatory and twenty (20) non-ambulatory clients. The facility is currently providing services to thirty-four (34) clients.
The facility does not have a swimming pool/body of water. LPA observed activities room with games, puzzles and other activities, computer lab and gym for clients.
Cleaning agents and other toxins are locked away. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to clients. Common area furniture was also checked. The common area is neat and clean.
The facility's last fire inspection was conducted on 04/02/2024. The most recent disaster drill was completed 03/07/2024. Fire extinguishers are located throughout the facility, observed to be full and last inspected on 05/09/2024. The facility has enough hygiene products available to the clients. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 113.7°F. LPA observed a complete first aid kit.
No health and safety hazards noted during the visit.
