Facility Evaluation Report

Licensing Program Analyst (LPA) Huma Rahimi met with Imelda Villanueva - Executive Director for a case management visit. The purpose of the case management visit is to address a deficiency observed during the course of investigation control # 31-AS-20241220150010. Deficiency observed was not alleged however are related to the complaint.

During the course of the investigation control # 31-AS-20241220150010 the following was observed:

It was discovered that the licensee failed to properly reassess and conduct a ongoing reappraisals and or needs and service plan for change in condition for resident #1(R1) in timely manner.

Deficiency issued and appeal rights explained.

Exit interview conducted and copy of this report signed and delivered.
