Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the house manager, Genesis Zuniga, and explained the reason for the visit. The administrator, Dania De La Riva, was notified,
At approximately 9:45am, LPA took a tour of the physical plant. The smoke alarms and carbon monoxide are dual, hardwired and interconnected. There are three (3) fire extinguishers. One is located by the kitchen, one in the laundry room, and one in the garage. The charge date is December 23, 2024. The last earthquake drill was conducted on 05/07/25. The last fire drill was conducted on 06/11/25.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives were stored in a locked drawer in the kitchen. Cleaning supplies were stored locked underneath the kitchen sink.
Bedrooms: There are four (4) bedrooms designated for residents' use. All four bedrooms are private. Bedroom #3 has the fire clearance for non-ambulatory. Bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There are three (3) bathrooms designated for residents' use. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 111 and 116 degrees Fahrenheit.
