The inspector’s account

Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Required - 1 Year annual inspection visit. LPA met with Co-Administrator Armin Karamian and explained the purpose of the visit. The adult day program has an approved fire clearance for fifty (50) ambulatory residents ages 18 and above. Currently there are no clients attending the program, since Licensee is awaiting the vendorization process by Frank D. Lanterman Regional Center.

LPA toured the facility with Co-Administrator at 10:50 am and observed the following:

The facility is a single-story structure with shared parking lot, three offices, storage rooms, 3 bathrooms. The facility has a large dining/activity area with a separate breakout area by the front door and a main central hall for gaming activities.

The required posting were observed by the front entry hall. The program site is clean, sanitary, and in good condition, all furnishings are brand new with ample seating to accommodate 70 clients and staff. There are three fire extinguishers, one in the kitchen pantry, one by the front entry and one by the rear exit door. All the fire extinguishers were purchased on May 1, 2025.

Kitchen: There is a refrigerator in the kitchen and a pantry area, next to the kitchen. There are no sharp kitchen utensils currently and there are no perishable/non-perishable food supplies, since there are no clients in care.

Storage rooms: There is a storage room next to the rear exit. There is another locked storage area in the large bathroom, where all the cleaning solutions are stored, inaccessible to clients.
