The inspector’s account

Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Required - 1 Year annual inspection visit. Upon their arrival, staff on duty was Karine Maloyan/Care Staff. LPA explained the purpose of the visit. Administrator Elina Melkonian arrived at 8:45am. The program is vendorized through the Frank D. Lanterman Regional Center. The Adult Residential Facility is licensed for 6 non-ambulatory adults, ages 60 and above, one of whom may be bedridden. Facility has hospice waiver for for 6 residents. Current census is 6 residents, two of whom are ambulatory.

LPA toured the facility with Administrator at 9:15 am and observed the following:

Required postings were observed in the hallway and office area. The front entry is the main exit door and there two exit doors leading to the backyard. The physical plant appeared to be very clean and sanitary. The fire extinguisher is located in the kitchen and was recently purchased on 03/21/2025. Facility conducts quarterly fire and safety drills, the last fire drill was conducted on 05/04/2025. The last earthquake drill was conducted on 06/16/2025. Facility is equipped with two fire doors, one in living room and one in kitchen.

The dual smoke alarms and carbon monoxide detectors are hardwired and interconnected. At 9:59 am the smoke/carbon monoxide alarms were tested and observed to function properly.

Common Areas : Include a living room and dining area. The common areas were properly furnished with ample sitting/tables for appropriate number of clients. Television set and activity/gaming areas were observed in the living room.
