Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted an Annual Required visit and inspection of the facility. LPA met with staff, Francisco Madarrte, and explained the reason for the visit. Administrator Lourdes Macatangay was contacted and arrived shortly after.
At approximately 09:25 am, with the assistance of staff, LPA took a tour of the physical plant. The following were observed
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility, properly stored. Knives were stored in a locked cabinet. The kitchen was observed to be clean and neat. The laundry area is located next to the kitchen. Laundry toxins were locked and inaccessible to clients.
Fire Alarm: The smoke and carbon monoxide detectors are hardwired, interconnected, and observed to be operational. Fire extinguishers are located in the laundry area and between the Clients' rooms. Fire extinguishers observed to be full and last inspected on 03/19/2025.
Bedrooms: There were three(3) bedrooms designated for clients' use. All bedrooms are designated for private use. All three bedrooms, in use by clients, were properly furnished with appropriate beddings and linens, with sufficient lighting
