The inspector’s account

Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Required - 1 Year annual inspection visit. LPA met with Administrator - Edward Parker, Jr and Program Director - Marinela Yammine. LPA explained the purpose of the visit. The Adult Day Program (ADP) is licensed for 60 ambulatory Developmentally Disabled Adults, ages 18 through 59. The program is vendorized through the Frank D. Lanterman and North Los Angeles Regional Centers. Currently there are 32 clients attending the program. The program does not manage medication but does provide breakfast and lunch to the clients. During today's visit, LPA utilized the Compliance and Regulatory Enforcement (CARE) tools.

At approximately 11:40AM, LPA toured the facility with Program Director and observed the following:

The facility is a single-story structure with its own parking lot and consists of a reception area, three offices, three storage rooms, five program breakout rooms, locker area for client personal storage, kitchen/all purpose room, conference room, staff break room, basket ball courtyard and a shaded patio area. There is a Children's Wing area in the building, which is not part of the program and is currently vacant.

There is one central entry point located at the back, that has been designated to screen all participants, staff, and visitors. There are five exits but currently the all purpose room door is used as the main exit. There are several pull switches for the smoke alarms, located in the hallways. There are three fire extinguishers, one in the kitchen, one in the hallway and one by the exit door. All the fire extinguishers were charged on August 5, 2025.
