Facility Evaluation Report

Licensing Program Analyst (LPA) Mariana Agban conducted an unannounced continuation of the annual inspection. LPA met with staff Alexander Calinog and disclosed the reason for the visit. Administrator Designee arrived shortly after Giovani Angeles. LPA reviewed Medication and Medication Records for proper documentation LPA also conducted a file review of staff records to ensure forms and training are up to date and compliance with licensing forms. LPA, with the assistance of the staff, reviewed cash resources for five residents. Logs and cash resources were consistent. LPA also reviewed six (6) out of 6 residents' files. LPA observed that Residents 1 and 2 have incomplete physician reports. Residents 2 and 3 have an incomplete form of LIC 601. Administrator Designee was advised and will provide the requested documents promptly.
