Facility Evaluation Report
Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Annual Required visit and inspection of the facility. LPA met with Administrator/Christine Lekhlyan and explained the reason for the visit. The facility is licensed to serve six (6) non-ambulatory clients and is approved for six (6) hospice waivers. Current census is three (3). Clients receive services from the Frank D. Lanterman Regional Center.
LPA asked for copies of staff and resident rosters and files. At 9:15 am, with the assistance of Administrator LPA toured the physical plant and observed the following:
Mandated postings were observed by the entry door and in the office area. Smoke/carbon monoxide detectors are interconnected. At 9:34 am, smoke/carbon monoxide detectors were tested and observed to be functioning properly. There are two (2) fully charged fire extinguishers, one in the kitchen and one by the rear exit door; both fire extinguishers were serviced on 03/24/26. LPA observed fire sprinklers throughout the facility. There are two (2) exits, one in the dining room and one in the hallway, leading to the backyard. Facility uses the main entry door as emergency exit. All exit doors were equipped with functional alarms. Facility conducts monthly fire/earthquake drills; the last fire drill was conducted on 03/13/26 and last earthquake drill was conducted on 02/26/26.
Kitchen: The kitchen appliances consist of a refrigerator, stove/oven, dishwasher, microwave. All appliances and fixtures were observed to be clean and in functional condition. Knives and sharp objects were stored in a locked drawer in the kitchen. LPA found sufficient supply of perishable foods for two (2) days and non-perishable foods for seven (7) days. The kitchen cabinets were observed to have ample dishes and cook ware. Chemicals and detergents were locked in a cabinet underneath the kitchen sink. Properly labeled medications were locked in a kitchen cabinet. First Aid Kit with First Aid Manual were observed in the medication cabinet as well.
