Facility Evaluation Report

On July 23, 2025, Licensing Program Analyst (LPA) Alfonso Iniguez conducted a case management visit. LPA Iniguez met with Melon Rivera, the Executive Director, to explain the purpose of the visit.

During this visit, which was part of a complaint investigation (11-AS-20250716110807), it was discovered that the facility's elevator had been in disrepair since June 26, 2025. The facility had not reported this issue to the department.

Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8. See page D for more information.

Note: *Citations not cleared by the due date will be a $100 fine assessed for each citation until it is cleared. Civil penalties will continue to accrue until Proof of Corrections (POC) is cleared. *

An exit interview was conducted, and a copy of the Facility Evaluation Report was provided to Melon Rivera/Executive Director.
