Facility Evaluation Report
On 9/25/2025, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual require visit using the CARE Inspection Tool. LPA met with Malia Lindy /Director of Operations. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) age range 18 and above ambulatory only. Short term crisis residential program. Currently, the program has (5) clients.
The facility is a two-story home located in a residential neighborhood. The property consists of the following: 5 bedrooms, 11 bathrooms, staff offices, living room, kitchen, dining room, activity room, 2 therapy rooms, and a basement with media room. There is a pool with a fence which leads to the admission office lounge with a bathroom.
LPA Iniguez and the program director toured the physical plant. There were no obstructions on the premises. The pool was fenced and locked during the visit. LPA inspected a total of (4) bedrooms and (4) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the clients’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 113.5°F to 118.2°F, and the room temperature ranged from 76°F to 78°F.
