The inspector’s account

On 10/24/2024, Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Keith Dicke ( Nursing Manager) and the purpose of the visit was discussed. The facility is a short- term crisis residential program, licensed to served 6 ambulatory adults/elderly clients. Currently, the home has (3) clients. None the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility is a two story home located in a residential neighborhood. The property consists of the following: 5 bedrooms, 11 bathrooms, staff offices, living room, kitchen, dining room, activity room, 2 therapy rooms, basement with media room. There is a pool with a fence which leads to the Admission office lounge with a bathroom.

LPA conducted a records review of (3) client records, (3) staff records, and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (3) Client Medication Records and did not observed any discrepancies at the time of visit.

At 12:00 pm LPA, Nursing Manager (Keith Dicke) and Program Manager (Katie Lunngren) toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed , plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between at 111F.
