Facility Evaluation Report
On 5/11/26, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Keith Dicke, Director in Nursing, and explained the purpose of today’s visit. Director of Operations, Malia Lindy, later joined LPA for the visit. The facility is a licensed Short term crisis residential program, approved for six (6) ambulatory adults. Currently, the program has five (5) clients.
The facility is a two-story home with a main and a side attached house located in a residential neighborhood. The property consists of the following: four (4) bedrooms, ten (10) bathrooms, staff offices, living room, kitchen, dining room, activity room, two (2) storage rooms, three (3) therapy rooms, laundry room, basement/playroom including pool table, games and a bowling alley. There is a fenced pool and shaded patio area in the backyard.
LPA and Malia Lindy toured the physical plant. There is a gated locked pool on the premises. No obstructions found on the premises. LPA inspected four (4) bedrooms and four (4) bathrooms. The beds and bedding supplies were in good condition. Adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be operational and within Title 22 regulations. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged between 105.0°F and 120.0°F. A comfortable temperature was maintained in the facility.
During the visit, LPA observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored and inaccessible to clients in care.
