Complaint Investigation Report
The details of the complaint allege that the facility is not well maintained and filthy inside of the facility. On 7/08/2026, from 9:30am-12:00pm, the department interviewed staff (S1-S2). C1 could not be interviewed because they have passed away. 2 of 2 staff denied the allegation Facility is unkempt. All of the staff stated that the facility has always been well maintained. Staff also stated that the facility has not had any staff nor clients since January of 2026.
The department toured the facility and observed that all rooms were clean throughout the facility and did not observe any hazards or obstructions in the facility or exterior.
The department did not interview any clients because their census is zero (0). Staff stated they have not had any clients since January of 2026.
Based on interviews and observations, there is insufficient evidence to support the allegation that Facility is unkempt . Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated .
Allegation#2- Staff did not provide sufficient food items to resident in care.
The details of the complaint allege the facility does not serve appropriate meals for the clients in care. It was reported that the facility only served a client toast for breakfast and a bologna sandwich for lunch. On 7/08/2026, from 9:30am-12:00pm, the department interviewed staff (S1-S2) regarding the allegation. C1 could not be interviewed because they have passed away. 2 of 2 staff denied the allegation that Staff did not provide sufficient food items to resident in care. All of the staff stated that when the facility had clients, they were served three meals per day and had access to snacks throughout the day. They deny that any client was just given toast for breakfast and a bologna sandwich for lunch.
The department reviewed the menu and observed that the menu plan provided for a full breakfast, lunch, and dinner with a mixture of fruits, vegetables, meat, grains, and dairy.
The department did not interview any clients because their census is zero (0). Staff stated they have not had any clients since January of 2026.
Based on interviews and records reviewed, there is insufficient evidence to support the allegation that Staff did not provide sufficient food items to resident in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated .
No deficiencies were found and no citations were issued for this complaint investigation.
An exit interview was conducted with Clinical Director, Nicole Nehoraoff, and a hard copy of this Complaint Investigation Report was provided.
