The inspector’s account

On 09/19/25, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced required annual visit using the Inspection Care Tool. LPA met with Operations Manager, Deandra Garcia and explained the purpose of today’s visit. The facility is licensed to serve six (6) ambulatory short term residents. During the time of visit there are two (2) clients.

LPA and Garcia toured both inside and outside of the facility. The facility is a one-story structure located in a residential neighborhood. The facility consists of (3) client bedrooms, (3) bathroom, living room, kitchen, dining area, patio, and laundry room.

LPA toured the physical plant. There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the resident's personal belongings was observed. Bed linens, comforters, and bath towels were available during the visit. Bathrooms were operational, with water temperatures measured at 105.7 degrees Fahrenheit. A comfortable temperature of 70°F was maintained in the facility.

LPA observed that the facility was furnished at the time of the visit. Storage areas for personal hygiene and sharps objects were inaccessible to residents. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. A fire extinguisher was charged. A review of the Medication Administration Records (MAR) revealed that it was in place.

A working landline phone was operational.
