The inspector’s account

Licensing Program Analyst (LPA) Tena Herrera conducted an unannounced Annual Inspection. LPA met with House Lead Carina DeLaCruz and explained the purpose for today’s visit, shortly after Administrator Rommel Gonzales arrived to assist with visit. The facility is licensed to serve 4 Ambulatory Adults ages 18-59 years.

This is a single-story home located in a residential area in Bellflower, Ca. A tour of the facility includes: living room, dining area, kitchen, 4 bedrooms, 2 bathrooms, staff room that has its own entrance through the back yard with a full bath and is separate from clients main living area, there is a front yard, back yard and detached garage with laundry area.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: The facility maintains the required Infection Control Plan.

Physical Plant & Environment Safety: LPAs toured facility, clients’ bedrooms were checked and closet/drawer space to accommodate each client comfortably was available. The backyard is free of debris/hazards and the outdoor and passageways are free of obstruction. Hygiene products are readily available for clients. The hot water temperature was tested and was within the required range of 105-120 degrees F. All storage areas for cleaning solutions, toxins, knives, and hazardous items are kept in a locked cabinet and are inaccessible to clients. Smoke detectors and carbon monoxide detectors are operable and in compliance. There fire extinguisher was observed and is fully charged.

Operational Requirements: Staff have proper training to meet the needs of the clients in care.

Staffing &Personnel Records-Training : There appears to be sufficient staffing at all times in the facility. Staff files are maintained in a secure location. LPAs reviewed 4 staff files during today’s visit, files reviewed contained the following: Criminal Background Clearance, First-Aid/CPR/AED/CPI and sufficient on-going training. Rommel Gonzales maintains a valid certificate that expires on 3/21/26 (and confirmed he is in process of renewal). (Continued on LIC809-C)
