Facility Evaluation Report
Licensing Program Analyst (LPAs) Tena Herrera and Gaby Castro made an unannounced visit to the facility regarding updated fire clearance and change of ambulatory status. LPA met with Administrator Rommel Gonzales and explained the purpose for the visit.
Applicant completed the change of AMB/NON-AMB status and clearance from the fire department was granted on 9/2/25; for 3 Ambulatory and 1 Non-Ambulatory clients. The 3 Ambulatory Clients will be in bedrooms 1, 2 and 3 (1ea). Bedroom 4 is approved for 1 Non-Ambulatory Client. (this is an update to the approved license of 4 Ambulatory only clients)
LPA toured the facility and observed the following:
Ramps: Exit from room #4 has a ramp leading to the back yard, and there were no obstructions in the pathway.
Doors: Door widths, within room #4 appear wide, to ensure accessibility for wheelchair and/or walker and each room has their own exit doors.
Grab-Bars: the facility does not have the required grab bars in the clients restrooms, Administrator to submit photos via email to LPA once grab bars are installed. (grab bars must be near toilets and in showers)
All entrance/exits, doorways and ramps had clear walkways.
No deficiencies observed during today's visit, exit interview held and a copy of this report was emailed to Rommel Gonzales.
