Facility Evaluation Report

An informal conference was conducted and held today in the Monterey Park Adult and Senior Care Regional Office. The purpose of this informal office meeting is to discuss the facility closure.

Present in this meeting: Araceli Ramirez (Regional Manager for Monterey Park Office); David Sicairos (Licensing Program Manager), Tena Herrera (Licensing Program Analyst), Stanley Kaniel (Licensee/Administrator) and Marilyn Nguyen (Buyer).

The Department was made aware of possible closure/change of ownership with a target date of end of November 2023. The following Title 22 Regulation Sections were discussed and materials provided during the meeting;

PIN 18-17-ASC Facility Closure Requirements

Stanley Kaniel and Marilyn Nguyen were made aware of the three closure phases. Phase 1 (draft of the closure plan along with eviction notice, submission of change of ownership application), Phase 2 (following approved closure plan, notifying placement agencies along with LTCO, SS Administration and transitional plan for the new buyer), Phase 3 (notification of change of ownership).
