Facility Evaluation Report
Licensing Program Analyst (LPA) Tena Herrera conducted a Case Management Visit-Deficiencies stemming from initial complaint investigation conducted on 05/27/25, complaint control # 28-AS-20250523123912 LPA Herrera met with Rana Chapkhaneh and explained the reason for visit.
During complaint investigation dated 5/27/25 LPA was reviewing staff files and during review asked for Staff #4 (S4) file, S1 stated that they did not have the file at the facility and was not exactly sure what location it is at, file was not provided to LPA in a timely manner during visit. This is a repeat violation form a citation issued during the annual inspection on 8/5/24 and civil penalties will be assessed during todays visit.
Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiencies observed during the visit are documented on 809D. Exit interview held and a copy of the report along with appeal rights were provided.
