Facility Evaluation Report
Licensing Program Analyst’s (LPA’s) Christian Gutierrez and Gabby Castro conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with DSP worker Wilma Madrigal at approximately 12:50 PM and explained reason for visit. Administrator Emmanuel Madrigal arrived shortly.
The facility is licensed to serve four (4) ambulatory adult clients aged 18 to 59. Currently, there are two (2) clients in placement. All clients residing at this facility receive case management services provided by Harbor Regional Center. The facility is a two-story home located in a residential area in Bellflower, Ca. Facility is licensed for the first floor/bottom floor only. A tour of the facility includes living room, dining area 2 client bedrooms, 1 client bathroom, kitchen, laundry room, detached garage. Upstairs has two bedrooms and 1 bathroom that is for staff use.
LPA toured the facility and observed the following: All client bedroom has the required furniture and bedding. There is extra clean linen and towels in laundry room. Smoke detectors/carbon monoxide detectors were observed throughout facility. The facility has one (1) fire extinguisher which is kept in kitchen and is fully charged. Cleaning supplies and toxic substances are inaccessible locked in cupboards in kitchen cabinet and laundry room. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 45 degrees F. LPA observed a sufficient supply of 2 days perishable foods and 7 days non-perishable foods were in the kitchen. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The facility does not have a swimming pool or large body of water. There is a shaded seating area for the residents located in backyard.
