Facility Evaluation Report
Licensing Program Analyst (LPA) conducted an unannounced Case Management visit to the facility to check if the facility was abiding by the terms of the facility’s fire clearance. The LPA was met by Caregiver Staff 1(S1) and the purpose of the visit was discussed. Administrator Mellanie Aikman was contacted by phone and joined the visit shortly after. Licensee Micheal San Diego was contacted by phone during the visit. The facility is licensed to serve Developmentally Disabled Adults 18 to 59 years old. Three (3) ambulatory and (2) non-ambulatory clients.
The facility is a single-story home located in a residential area in Bellflower, CA. LPA toured the common areas and observed no health and safety concerns.
LPA asked and obtained copies of current client and staff rosters and copies of Physician’s Reports for C1 and C2. LPA was shown by staff which rooms were occupied by which clients. C1 and C2 Physician’s Reports designates C1 and C2 as being non-ambulatory.C1 occupies Room 3 on the facility sketch and C2 occupies room 4 on the facility sketch. Room 3 is a designated as a staff room for an ambulatory staff member and Room 4 is designated on the facility sketch as being for an ambulatory client. The facility is not operating within the fire clearance parameters and limits of the license. Non-ambulatory clients are occupying rooms to be used by ambulatory persons and a client is occupying a room that is to be used by a staff member. Two deficiencies were cited. See 809-D.
Per Title 22 Regulations deficiencies were cited today. An exit interview was conducted with Administrator Aikman and a copy of this Licensing Report was provided along with Appeal Rights.
