The inspector’s account

During today’s visit, LPA toured the facility and obtained the Staff roster and Client roster via email.

The investigation reveals the following: Regarding “Staff hit client in care”. It is alleged that S3 hit C1. The Administrator denied the allegation stating the incident was investigated a month prior by state licensing and the police department. According to the administrator S3 has never physically assaulted any of the clients. 3 out of 3 staff denied the allegation stating staff as never hit any of the clients. 1 out of 1 client denied the allegation, by stating staff has never assaulted them. The Police investigated and C1 stated to the police that they do not remember staff hitting them.

Based on LPA's interviews, investigation revealed: Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
